Make sure you fill out all the information to the best of your knowledge and do not leave any questions unanswered as this could delay the order. In order for us to expedite and ensure accuracy on orders, please take the following steps:

1. Fill out the required forms

 

• Print and fill out the Hygeia Intake Form

 

 

 

 

• *In addition, fill out the Switch Of Provider Form if your patient is switching from another Durable Medical Supplier to Hygeia Medical Supplies and Services, Inc.

2. Indicate what type of equipment or supplies you are ordering for your patient
